Join us for this interactive discussion which will identify the benefits of collaboration between ADS and Hospice/Palliative Care, and enhancing end of life care and support in the ADS setting.
A portion of the program will delve into the results of an Easter Seals initiative "Enhancing Community-Based Options for End-of-Life Care." Easter Seals project is creating tools and training resources to help adult day services personnel and hospice/palliative care personnel across the country better understand each other's roles and integrate clinical practices, programs, and policies.
This program will also address how policy is being approached at the state and national level to reduce barriers and promote collaboration between these two vital communities of care.
No one should have to approach death without pain management, compassion and support. This initiative will help begin to make a real difference for many families during such a difficult time.
